Does Part D of Form 671 need to be completed if I already received a Davis-Bacon certificate?

Yes, Part D is completed by the registration agency as part of every successful registration. Receiving your Davis-Bacon certificate is actually proof that Part D has already been signed and the apprentice is officially registered in the DOL’s RAPIDS system. You do not need to request the Part D-completed version unless you have a specific need for that exact document in your records.


Why the Davis-Bacon Certificate Means Part D Is Already Done

The Davis-Bacon certificate is only issued after the U.S. Department of Labor (or recognized State Apprenticeship Agency) has:

  • Reviewed and approved the Apprenticeship Agreement (Form ETA-671)

  • Officially registered the apprentice in RAPIDS

  • Completed and signed Part D with:

    • Official registration date

    • Apprentice Registration Number

    • Agency signature or stamp

In other words: No signed Part D = no Davis-Bacon certificate. If you have the certificate, you already have confirmation that Part D was completed and the full registered agreement is securely stored in the Department of Labor’s official records.


The Two Versions of Form ETA-671

During registration, two related versions exist (they are not merged into one combined document):

  • Version 1 — Your submitted agreement Signed by the apprentice, employer, and sponsor (Parts A, B, and C completed). This is the version you typically have in your files.

  • Version 2 — The official registered agreement Same content as Version 1, but now with Part D completed and signed by the registration agency. This version lives in the DOL’s secure system as the definitive record of registration.

Because the DOL maintains this official record, the Part D-completed version is not automatically sent to employers or sponsors during the standard Apprentix registration workflow.


What Counts as Proof of Registration?

For compliance needs, including IRA tax credit documentation, prevailing wage verification, project reporting, and most audits, the following are universally accepted:

  • Davis-Bacon certificate

  • Registration confirmation email/notification from Apprentix or the agency

  • The apprentice’s unique Apprentice Registration Number

You’re already in compliance with these items in hand.


When & How to Request the Part D-Completed Version

You only need to request the official registered copy (showing Part D) if you have a specific requirement—for example:

  • Certain third-party audits

  • Particular project or funding documentation

  • Internal record-keeping policies that require the agency-signed version

If that applies, Apprentix is happy to retrieve it directly from the Department of Labor for you.

Steps to request it:

  1. Reply with the Apprentice Registration Number for each apprentice you need the registered Form ETA-671 for.

  2. We’ll submit the request(s) to DOL on your behalf.

  3. Requests are processed individually per apprentice to ensure accuracy and speed.
